What is the COVID-19 Return To Work Form?

All employees returning to the workplace need to complete a return to work form at least 3 days prior to attending the workplace.

Employees need to inform employers about any circumstances related to coronavirus  and take part in any training the employer is providing.

The pre-return to work form needs to be completed at least 3 days before employees return to work. This form is is company specific and needs to be provided by the company to the employee.

Employees will need to confirm that they do not have symptoms of COVID-19, are not self-isolating, are not waiting on results of a COVID-19 test and have not been in contact with someone who has the virus.
